Combine bourbon, tepache, lemon juice, and Falernum in a cocktail shaker; fill shaker with ice. Shake until outside of shaker is frosty, about 30 seconds. Strain into a chilled double old-fashioned glass filled with crushed ice. Top with bitters, and garnish with orange twist.
